Kootenay Boundary Virtual Health Patient Survey 2020

This very short survey is intended for residents of the Kootenay Boundary who have had a health care appointment by phone or video since the beginning of March 2020.
It can also be completed by people who have helped a child or another person participate in a phone or video appointment since the beginning of March 2020.

Virtual Health, sometimes called Telehealth, connects patients with care providers remotely by phone or by videoconference. During the COVID-19 pandemic there has been an increase in the use of Virtual Health/Telehealth by health care providers. We would like to explore your Virtual Health experiences in this survey.

Your participation is voluntary and anonymous. You may stop answering this survey at any time, or skip any question that you do not wish to answer. We will not collect any personal information in this survey.

Results will be utilized by the Kootenay Boundary Division of Family Practice for project planning, project evaluation and improving the supports offered to Kootenay Boundary providers to support the utilization of virtual health. Aggregated results may be shared with health system leaders and organizations.
