Comox Valley Patient Experience Survey

Your doctor or nurse practitioner wants to know about your experiences at your primary care clinic (the clinic you go to regularly). This survey should take about 10 to 20 minutes to complete.
 
Your participation in this survey is voluntary and anonymous. We encourage your response but do not require you to respond.  You can stop answering at any point. You can skip any questions you do not wish to answer. You will not be asked for any identifying information, such as your name.
 
Completed surveys will be analyzed by a Comox Valley Division of Family Practice evaluator. Results will be used to improve care at your clinic and may be shared with other health care organizations, such as Island Health. If you have any questions about this survey or how the results will be used, please contact the Division of Family Practice evaluator at cv_evaluator@divisionsbc.ca.

Island Health and the Comox Valley Division of Family Practice will not collect, use, or disclose personal information through this survey. Please do not provide any of your personal information or any third-party information (i.e. information about others) in your survey responses. The data collected via this survey will only be stored online in Canada on Divisions BC servers.

Primary medical care is the care you receive at a family physician's clinic or at a walk in clinic.
